LANDSCAPE ENHANCEMENT REQUEST FORM

 

In accordance with the Declaration of Covenants and Restrictions of Shadow Lake Village Condominium Association, Inc., and amended By-Laws, I hereby apply for written consent of the Architectural Control Committee to make the following landscape enhancements (Describe in detail – Include drawing):

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

I acknowledge that any material I purchase and plant on common property becomes the property of the Association.  The property owner will assume expenses for future maintenance of plantings.

NOTE:  Contractor must furnish proof of Liability and Workers’ Compensation insurance to Association before commencing work.  For work to be done by homeowner, a “Waiver of Workers’ Compensation and Liability” form must be filed before work commences.

When residents wish to do any kind of plantings on “common property” (not foundation plantings), they are required to submit a “Landscape Enhancement Request Form” detailing the location and type of plant.

 

The exact location of the intended planting and the name of the plant must be included.  Providing the exact location of these plantings is necessary so that the Architectural Control Committee can determine if the plantings will interfere with above-ground structures such as buildings, sidewalks, vehicular visibility; and whether they will interfere with any underground utilities.  Please also include the expected dimensions of the planting when it matures.
